EFCH Community Survey

To access the assessment form, click on the form title shown in blue above.

To answer the assessment questions do the following:

1. For open-ended or literal questions click in the answer box provided and type your answer.
2. For multiple choice answers click the radio button beneath your answer. If you change your mind, click on the preferred answer. The original answer will disappear.

If you are not satisfied with your answers and want to start over, click the “Reset” button at the bottom of the survey which will erase all your answers.

When you have completed the survey click the “Submit” button at the bottom of the survey. A message will appear thanking you for the taking the survey.

To logout close the CFMA website or your internet connection.
